What digital smile design is
Digital Smile Design (DSD) is a planning approach that combines facial photographs, intraoral scans and CAD software to design a patient's new smile before any tooth is prepared. Instead of asking the patient to imagine the result, the clinician shows a realistic preview built on the patient's own face and teeth.
The concept started as a slide-based protocol using reference lines drawn over photos. Today the same planning happens directly in 3D: the facial scan or calibrated photo is aligned with the intraoral STL, and the new tooth shapes are designed in real proportions, not estimated on a flat image.
How the workflow runs
A typical digital smile design case moves through a predictable sequence. The quality of the input material decides how accurate the preview is, so photos and scans need to be taken with a consistent protocol.
- Full-face and retracted photos with a visible reference (such as a cheek retractor or calibrated ruler)
- Intraoral scans of both arches plus a bite registration
- Facial scan or calibrated photo aligned to the STL data
- Digital wax-up of the planned teeth, checked against lip line and smile curve
- Patient approval on the preview before any preparation begins
Why it changes the case conversation
When a patient can see the planned result on their own face, acceptance of aesthetic treatment plans rises and misunderstandings fall. The preview also becomes a working tool for the lab: the approved design is the same geometry used for the mock-up, the provisional and, after preparation, the final restorations.
That continuity is the real value. The smile design is not a marketing image — it is the blueprint that drives reduction guides, temporaries and the definitive crowns or veneers.
From preview to final restorations
Once the patient approves the design, the same project continues into production. The approved wax-up is the starting point for the final crown and veneer designs, so the result in the mouth matches what the patient approved on screen.
